Program Overview

SSM Health Saint Louis University's [www.ssmhealth.com] Hospital Violence Intervention Program (HVIP) program supports patients by bridging the gap between hospital care and outpatient recovery. It provides personalized assistance to ensure continuity of care, connecting individuals to community resources, engaging family support in the healing process, and promoting better health literacy. The program is dedicated to fostering comprehensive recovery and empowering patients for a healthier future.

The program is funded by the American Rescue Plan Act (ARPA).

Program Eligibility

Residents of the City of St. Louis over the age of 31 are eligible for services. 

Patients in police custody during hospitalization and/or incarcerated upon hospital discharge will not be eligible for services.
