Re-Entry Community Linkages (RE-LINK)

The Reentry Community Linkages program connects formerly incarcerated individuals to community-based organizations that provide access to care, substance use treatment, and social service resources that address social determinants of health needs.

Program Overview

Services rendered by Reentry Community Linkages are led by Community Health Workers who are focused on addressing three primary goals:

  1. Improving the coordination and linkages among criminal justice, public health, social service, and private entities to address health care and health care access
  2. Reducing the health disparities and recidivism among the justice-involved population in the City of St. Louis
  3. Increasing the accessibility of services and resources to those recently incarcerated

Participants have access to the following resources/services:

  • Transportation assistance
  • Clothing assistance
  • Work clothing/supply assistance
  • Household goods Source documentation assistance
  • Utility assistance
  • Medication and medical supplies assistance
  • Legal fines and supervision related fees

The program is funded by General Revenue.

The program is operated by the Saint Louis Integrated Health Network

Program Eligibility

Services provided by Reentry Community Linkages are available to any person returning to St. Louis City after incarceration in the City Justice Center, County Justice Center or Missouri prisons, as well as individuals under the supervision of the Missouri Board of Probation and Parole.

Please Note: The program will not accept individuals who are experiencing homelessness and are unable to identify supplemental support.
