FY 2023 Departmental Forms Available

Forms should be completed by departmental accounting contacts and returned with original signatures by June 30, 2022.

Fiscal year 2023 forms are now available. The Comptroller’s Office requires each department complete:

  1. An authorized signature form for each fund/center for which they are budgeted.
  2. A department contact information sheet.

Return completed forms with original signatures by June 30, 2022 to:
