Progress and Reporting Requirements

• OAs must demonstrate program is progressing as planned in the original contract.
• OAs must demonstrate program is clearly meeting a goal in the Consolidated Plan, and has quantifiable data available for performance reporting.

Programmatic Reports

• due on the 1st of each month and no later than the 10th calendar day of each month following the monthly reporting period.
• must be submitted on your agency's programmatic report form with correct information.
• must be signed by the person who prepared the report and the OA's authorized signatory.
• must include ethnicity and race data and other required additional documentations, if applicable.
