Process for obtaining medical records from the Department of Health.

Overview

Medical Records including immunization records, lead screening results, and Tuberculosis are available from the City of St. Louis Department of Health. 

Birth Certificates and other vital records are managed by the City of St. Louis Recorder of Deeds and Vital Records Registrar.

Preparation

When you call (314) 657-1548 to request records, you may be asked the following questions to assist with your request: 

  • What type of records are you seeking? (immunization, lead screening results, tuberculosis, etc.)
  • What is your name?
  • What is your date of birth?

Instructions

Medical Records services are available through walk-in or by appointment.

For medical records, call (314) 657-1548 to discuss your request with a Department of Health employee. If records are available, you will be given a time to retrieve the records from the Department of Health office at:

Fees

The fee is $10 / per record (cash only, please have exact change).

What to Expect

After the Department of Health employee confirms the requested records are available, you will be given a time to visit the Department of Health offices to retrieve the records.
